In consideration of being permitted to participate in this bike ride activity (“Activity”), I,

the undersigned, agree as follows:

1. Photo/Video Consent and Release

I grant permission to Music City e-Bikes to photograph, film, or otherwise record my

participation in this Activity. I understand and agree that these images, videos, and recordings

may be used in promotional materials, websites, social media, or other publications without

compensation. I release all rights to any such media and waive any right to approve final use.

2. Assumption of Risk

I understand that participating in this Activity involves inherent risks, including but not

limited to falls, collisions, injuries, and exposure to natural elements. I voluntarily assume all

such risks, known and unknown, even those arising from negligence.

3. Hold Harmless and Waiver of Liability

I hereby release, waive, and hold harmless Music City e-Bikes, its officers, directors,

employees, volunteers, sponsors, and affiliates from any and all liability, claims, demands, or

causes of action arising out of my participation in this Activity, including personal injury,

property damage, or wrongful death, whether caused by negligence or otherwise.

4. Medical Treatment

I consent to emergency medical treatment if necessary during my participation. I am

responsible for any associated medical costs.

5. Acknowledgment of Understanding

I have read this agreement, fully understand its terms, and sign it freely and voluntarily. I

am at least 18 years of age, or if under 18, my parent/guardian has also signed.
